| Fluids Ward Consent Packet | This 4 page document includes all the forms necessary to transfer a patient to CCVSC for a fluids ward. Please fill out as much of the information as possible before faxing or delivering to our hospital. We appreciate your attention to this matter, if we do not receive all of these documents, we may not be able to commence a fluids ward stay.
| | Patient Referral Form | To send us a patient for care, please consider downloading and filling out this questionairre for us. We also have other sheets that may have already been distributed to you that will work as well. The more information we can gather on your patient, the better we can serve the patient, client, and you.
